This document is targeted towards instructors that wish to request public web server or MySQL databases for each member of a particular class. Once class resources are provisioned, the instructor of the course will have full access to any student site files and/or MySQL databases.

- Your class section number
- Any cross listed courses
- How the resources will be used. For example: each member of the class will create a WordPress page
